Stained Glass Registration Forms

There are now two ways to obtain forms to register your windows in the Michigan Stained Glass Census:

1) If you fill out the Preliminary Survey Form and submit it electronically, we will mail you forms so that you can register your stained glass windows. This is also an easy way to notify us about family, friends, etc. who would like to be contacted about registering their windows. Click here to fill out the online Preliminary Survey Form.

2) A PDF version of all of our forms with instructions is also available to download. You must have Adobe Acrobat to read it.
